What is that supposed to mean? A first what?

If "first" is an adjective/adverb in this case, there is missing the related noun/verb.
If "first" is a noun in this case, the "a" makes no sense to me.

Can someone enlighten me? Thanks in advance.

첨 = 처음 = first

So i think it's kinda literally translated, but i don't get the meaning.
